Teshima is starting out as a Gladiator and It's Difficult To Be A Tank, especially when your only partner is a Thaumaturge who just laughs from a distance, and neither of you have a single healing spell between you.
Your Thaumaturge revives you from the dead, a taunting smirk is the first thing you see when you open your eyes. Urgh. Sure you could switch classes but. Pride. Fool.
Go either straight out fantasy au, or them playing the mmorpg. I am happy with either. (You also don't actually have to have them die at any point--)

an alternative view to this. Teshima is a newbie Gladiator, and life is hard.
Why isn't Aoyagi with Teshima for their starter throes? Possible scenarios:
1. Aoyagi is a good kid who needs to keep up her gpa alongside all that death by cycling, you scrub.
2. Aoyagi chose Conjurer as a healer complement to Teshima's Gladiator, but Conjurers start out in green green Gridania, way far away from dusty desert Ul'dah. Whoops.
3. Aoyagi is lvl45-50+ and far away in patch 3.0++ lands, and Teshima is too (embarrassed) prideful to let Aoyagi carry him through leveling up.

One's an Arthurian Knight-Cyborg, the other's their Witch Enemy. They're entangled in eternal turmoil, a murderous battle, where the Knight strives to murder the Witch, and the Witch always revives the Knight to renew the cycle. And they fall in love, somehow. ...It's wonderful.
Bonus points if Miki or Manami are the Knight, and are really fascinated about their gradual intensification of cyborg-ness. Miki opening up her chest or arm chasis to peer, fascinated, at the inner workings and machinations. Nice.

Aya is the sensible one with a gun, keeping Miki safe from her own recklessness - that is, when it's not the other way round. "He looked like he needed a punch, Miki!" "Aya no."
Miyahara doing her best in keeping her place, in this post-apocalyptic society. A leadership role of sorts maybe. Iinchou as always, even now huh.
